Apply to a special education school: Documents required

• Child's student pass or Dependant Pass.
• Child's deed poll, if your child has changed their name.
• Parents' or guardian's passport and Employment Pass, if applicable.
• Certificate of legal guardianship, if applicable.
• Divorce and custody documents, if applicable. 

Reports

• Parent report. You can download the report template, complete and submit it in your application. Or you can fill it in directly on the online application form.
• Report book or progress reports.
• School report by a teacher most familiar with your child's learning needs.
• Medical report by a doctor who is familiar with your child's medical history and diagnosis. 
• Psychological report by a qualified psychologist.
• Workplace Literacy and Numeracy scores for applications to Metta School and Delta Senior School for students aged 16 to 18 years old.
