Apply to a special education school: Documents required
Find out what documents and reports are needed when you apply to a special education (SPED) school.

You must upload the soft copy of these documents and reports when you apply to enrol your child in a SPED school.
Personal documents
- Child's birth certificate.
- Child's deed poll, if your child has changed their name.
- Parents' or guardian's NRIC or passport.
- Certificate of legal guardianship, if applicable.
- Divorce and custody documents, if applicable.
- Parent report. You can download the report template, complete and submit it in your application. Or you can fill it in directly on the online application form.
- Report book or progress reports.
- School report by a teacher most familiar with your child's learning needs.
- Medical report by a doctor who is familiar with your child's medical history and diagnosis.
- Workplace Literacy and Numeracy scores for applications to Metta School and Delta Senior School for students aged 16 to 18 years old.
- Psychological report by a registered Educational or Clinical Psychologist listed in the Singapore Register of Psychologists.
Use the new Customised Psychological Report Template for applications and transfers (from other SPED schools) to the following SPED schools offering the National Curriculum (NC):- Pathlight School
- St. Andrew's Mission School
- Anglo-Chinese School (Academy)
More information about the school and the social service agency leading it will be updated on the MOE website by February or March 2025.
